Osteotomy to Correct Malalignment

The word Osteotomy means a cut (tomy) in the bone (osteo), and refers to an operation performed when a bone is malaligned, and therefore creates an adverse mechanical environment.  This can manifest in pain, or accelerated wear of a joint surface.  The knee requires a specific angular or rotational alignment of the bones in order to transfer the weight of the body evenly and with stability.

Angular malalignment has been shown to be strongly associated with osteoarthritis.  For this reason in early degenerative disease, the malalignment can be corrected as a means of slowing the progression of the arthritis.

This angular correction is usually performed on the tibia bone for a bowed or varus deformity, whereas as a knock-knee or valgus deformity is most commonly corrected at the femur bone.

In both cases the angular correction is made, and the bones are held together with screws and plates, or an external fixator, until the bone heals.
